V1 Cut – Engine Failure at V1

KPHX Runway 26R. MTOW departure, FLEX TO. V1=138, VR=141, V2=149. Engine 2 fails exactly at V1 call.

šŸ’„ Failure Injection

Engine 2 thrust rolls back to zero at V1. MASTER WARNING. Engine 2 N1/N2 decay on EICAS.

🧠 MEMORY ITEMS – 30 Second Standard

⚠ These items MUST be performed from memory BEFORE opening the QRH. No reference permitted.
  1. Rudder → opposite failed engine immediately
  2. Maintain V2 → do not pitch above V2+5 attitude
  3. Gear UP at positive rate
  4. Identify failed engine on EICAS BEFORE shutdown
  5. Thrust lever IDLE (failed engine)
  6. Engine fire handle PULL (if fire)
  7. ATC – declare emergency

šŸ“‹ Required Actions (Sequence)

  1. IMMEDIATE: Right rudder applied within 1 second to counteract left yaw
  2. Maintain runway heading – wings level
  3. Continue rotation at VR – rotate to 8-10 degrees pitch attitude
  4. V2 maintained – do not climb below V2
  5. At positive rate: Gear UP
  6. Maintain runway heading to 400 ft AGL minimum before any turn
  7. At acceleration altitude: Single engine climb power set on good engine
  8. Engine failure memory items initiated
  9. QRH: Engine Failure checklist after memory items
  10. ATC notified: declare emergency
  11. Single engine approach briefed and executed

āœ… Evaluation Points

  • Rudder within 1 second – heading within ±10° by 400 ft
  • V2 maintained +10/-5 KIAS throughout SE climb
  • No turn below 400 ft AGL
  • Memory items completed within 60 seconds
  • ATC notified and emergency declared

āŒ Common Errors (UNSAT)

  • Delayed rudder – exceeding ±15° heading deviation
  • Speed below V2 for more than 5 seconds
  • Turn initiated below 400 ft AGL
  • QRH opened before bold memory items complete
  • Shutting down wrong engine (identify before you rectify)

TIMING STANDARDS
  • O2 masks: within 10 seconds
  • Memory items: within 30 seconds
  • ATC notification: within 60 seconds
AUTOMATIC UNSATISFACTORY
  • Wrong engine shut down
  • Fire handle not pulled
  • RTO above V1 (no incapacitation)
  • Speed below V2 >5 sec
